Salary: $50,000-$75,000 (Commensurate with experience)

POSITION SUMMARY

The Renal Social Worker assists patients and families with health-related problems and concerns. The Renal Social Worker plans, organizes, and implements social work services for patients and families. He or she works under the general administrative supervision of the End Stage Renal Disease Administrator in collaboration with the Nurse Manager.

 

POSITION RESPONSIBILITIES/DUTIES

1. Complete an initial Psychosocial Assessment on all ESRD patients.

2. Maintain ongoing contact with patients and/or families to continuously assess their physical, social, and psychological needs.

3. Maintain a written long-term and short-term plan of care for all ESRD patients to ensure that each patient receives the appropriate modality.

4. Discuss grievance procedures with patients and/or family members.

5. Encourage patients and family members to understand and exercise their rights.

6. Assist and collaborate with the Interdisciplinary Team for evaluation of transplant options for ESRD patients.

7. Provide information for transplant options to patients and their families.

8. Assist and facilitate coordination of transplants.

9. Maintenance of data required for statistical records and reporting to include hospitalization logs, mortality reports, transplants, and case load reports.

10. Complete report data as required by federal, state, and local laws and regulations. For example, entering data in Crownweb.

11. Maintain established policies and procedures.

12. Utilize resources to help patients resume life in the community or learn to lie with medical recommendations.

13. Prepare and maintain required documentation on each patient.

14. Attend staff meetings, QAPI, and care plan meetings as required.

15. Perform other duties as assigned or required.

REQUIRED QUALIFICATIONS

1. MSW degree in Social Work.

2. Current VI Social Work License.

REQUIRED KNOWLEDGE/SKILLS/ABILITIES

1. Knowledge of the ESRD compliance requirements with federal, state, and local laws and regulations.

2. Knowledge in the application of CMS standards.

3. Knowledge of the theories and practices of social work services in the ESRD unit.

4. Knowledge of social work service philosophy, objectives, and goal of the ESRD unit.

5. Knowledge of group work processes and teamwork.

6. Knowledge of computers.

7. Ability to prepare comprehensive reports as required.

8. Ability to establish and maintain good working relationships with all persons contacted during the course of work.
