Accessing Medical Records:
Under HIPAA regulations (45 CFR §164.524), you have the right to access and obtain copies of your protected health information. Our Health Information Management (HIM) department is committed to processing your request efficiently. If exceptions apply, we will review them accordingly.

Requesting Medical Records
To ensure a smooth process, please follow these guidelines when requesting medical records:
Photo ID Required: Always present a valid photo ID when requesting your medical records.
Requesting for Someone Else? You must complete a Release of Information (ROI) form and provide both your ID and the patient’s ID.
Processing Time: Most requests are fulfilled within 24 hours, but processing may take 5 to 7 business days depending on the request.
Hospital Requests: Requests related to a hospital stay require an ROI form and are typically processed within 24 hours to 5 business days, depending on the length of stay.

Medical Record Amendments
If you discover any errors or incomplete information in your medical record, feel free to request a correction.
To request an amendment/correction, download and complete the “Request for Correction/Amendment of Protected Health Information” form.
Please Note:
Gov. Juan F. Luis Hospital & Medical Center may take up to 60 days from the date of request to respond. If we are unable to respond within 60 days, a written notification detailing the reasons for the delay will be sent to you with the new expected response date.
Completed amendment forms should be returned to the Medical Records/Health Information Department. You can submit the forms by mail to:
